The New Fund Request Form is to be used for Non-Research related purposes.

• A fund is created at the sole discretion of Financial Services.

• Please note that individual funds are not normally opened for amounts of revenue or expense less than $10,000.

Before filling out a form, SAVE it to your computer and then open it in Adobe Reader.
This will ensure that no data is lost when clicking on any of the links within the form.

Consult the checklist on the new fund request form before submitting the form.
